Your first visit to Maple Orthodontics will include:

1. a free consulation and professional diagnosis by our trained and specialized orthodontist, Dr. Sebastien
2. a detailed break down of what to expect for you or your child before, during and after treatment with us

Before you arrive to your first free consultation at our office, please print out and fill out the following forms to ensure that we are brought up to speed with any and all important needs and health information:

1. health history form
2. supplementary questionnaire
3. insurance form (optional)
